According to the Medicare Payment Advisory Commission, nearly one-fifth of Medicare beneficiaries (a total of about two million) discharged from the hospital return within 30 days. The readmissions are both planned and unplanned, and they may or may not be related to the original reason the patient came to the hospital. This course explores the factors that lead to readmissions, and how to prevent unnecessary readmissions in the future.
